Who are involved in the Virtual Ward?  A core team of Doctor, District Nurse, Social Worker will meet daily to perform ‘ward rounds’ for all Virtual Ward patients.  This means that they will discuss the care that you may need and what support they can give you while you are unwell at home.

The team may request support from other professionals for your care for example Occupational Therapists or Specialist Nurses.  These teams will be brought together for detailed discussions at weekly meetings to ensure everybody is aware of what care you require.  The team will discuss your needs with you on an ongoing basis to help you make decisions about your care.  Virtual Ward Leaflet
