Registering with the Practice

Please check that you are currently living within the practice boundary. The map of the practice boundary is defined by the red line on the map above. If you are living inside the practice boundary then you are able to register at the practice.

If you do not currently live within the practice boundary then you will need to register at a Doctors surgery which is more local. If you are unsure of which Doctors you can register with, then please contact Powys Teaching Health Board on 01874 711661.

Registration forms can be downloaded from the links below or you can call into Reception and ask for them.  If you have any difficulty in  completing the forms our reception staff will be happy to assist you.

There are two forms to complete for adults (health questionnaire and official NHS registration form) and just one (official NHS registration form) for children. For your convenience, the adult health questionnaire can be downloaded below. If the child has any ongoing or complex health issues then please complete a health questionnaire too.

Once this form is complete the registration process starts and we will be able to obtain your medical records from your previous GP. It takes an average of 48 hours to register patients but it can take many weeks for your medical records to arrive from your previous GP. Please attach a list of your repeat medication form to your registration form as this will help the Doctors and avoid any delay in obtaining your medication.

We offer health checks to newly registered patients. This includes checks such as a blood test and blood pressure. If you would like an appointment please contact the surgery to book.
