How do I book an appointment or speak to my doctor or nurse?

The best way to contact us is via our online form. Click the image below to access our online service

  • One of our clinicians will review your form the same day. A member of the team will then be in contact accordingly, depending on clinical urgency.
  • This may be up to 2 days. If you are unable to use the online form, then please contact reception, and they can complete this form on your behalf.
  • You are able to express within the form if you have a preference for a practitioner.
  • If you wish to make a digital request for routine matters outside of our form opening times then we have an email address clinical.enquiries.w96013@wales.nhs.uk. Please only use this for routine matters as we may take up to 2 working days to respond. If your request is urgent then please call the practice on 01597 824291.

Appointments We Offer

  • Appointments in advance to see a clinician of your choice
  • If your problem is more urgent, we will offer you an appointment that day. Requests are triaged by our clinical staff, and you will receive a call or text back with an appointment.
  • If you require advice, arrangements can be made for a Nurse or Doctor to return your call.  This is useful for discussing test results or to get some advice on your medication, etc.

Children 16 years and younger presenting with an acute issue will be assessed on the same day.

HOME VISITS

  • If you are unable to come to the Surgery and require a home visit, please telephone before 10.30am

Home visits are very time-consuming and wherever possible patients should attend Surgery.
